Description

This presentation will focus on managing anticoagulation therapy including choosing a vitamin K antagonist or direct oral anticoagulant, resuming anticoagulation therapy following a bleeding event, and managing anticoagulation during an invasive procedure including bridge therapy. The choice of an anticoagulation therapy should be guided by identifiable patient-specific clinical factors and patient preference.

Objectives

Objectives
- Point out patient -specific factors that will help clinicians determine preferred anticoagulation therapy options.
-Distinguish patients who should resume anticaogulation therapy following an episode of anticoagulaion therapy-related major bleeding.
-Describe how recent evidence has altered recommendations regarding the use of bridge therapy for anticoagulated patients undergoing invasive procedures.

Daniel M. Witt, Pharm.D. FCCP, BCPS

Daniel M. Witt, Pharm.D., FCCP, BCPS is Professor (Clinical) and Vice Chair of the Department of Pharmacotherapy at the University of Utah College of Pharmacy. He received the B.S. degree in pharmacy from the University of Utah and the PharmD degree from the University of Washington. He completed a Clinical Pharmacy Residency at the University of Washington and Harborview Medical Centers, Seattle, WA and an Advanced Residency in Primary Care and Family Medicine, University of Washington. Dr. Witt has authored or co-authored over 75 journal articles, seven book chapters, and given numerous lectures for continuing education programs. He was a panel member for the 2012 CHEST Consensus Guidelines for Antithrombotic therapy. Dr. Witt serves on the Board of Directors for the Anticoagulation Forum and is a member of the American College of Clinical Pharmacy, American Society of Health-Systems Pharmacists and the International Society on Thrombosis and Haemostasis.
