Description

This seminar will review the components of oxygen delivery and how oxygen delivery to the tissues may be compromised despite adequate delivery via the macrocirculation (i.e. loss of hemodynamic coherence). The mechanisms regulating blood flow through the microcirculation will be reviewed, as well as regulation of transvascular fluid balance and the pathophysiology that may impair microcirculatory blood flow. Although treatment approaches will not be discussed, methods to assess and monitor the microcirculation will be described, including their application and limitations.

Objectives

Definitions
- To review the definitions of microcirculation, microcirculation, and hemodynamic coherence, as described in this review.
Anatomy and Physiology
- To review the basic anatomy, physiology, and role of the macrocirculation and microcirculation.
Regulation of the Microcirculation
- To develop an understanding of the regulatory inputs to the microcirculation in the control of local blood flow.
- To understand the role and function of the endothelium, endothelial glycocalyx, erythrocytes, and smooth muscle cells.
Loss of Hemodynamic Coherence
- To develop an understanding of what is meant by loss of hemodynamic coherence.
- To understand how pathological changes in these components affect microvascular flow and transvascular fluid balance.
Assessment of the Microcirculation
- To understand various techniques for monitoring of the microcirculation
- To recognize the indication and limitations of various monitoring modalities for the microcirculation.

Julie Dechant, DVM, MS, DACVS, DACVECC

Dr. Julie Dechant obtained her DVM from the University of Saskatchewan. She did a private practice equine internship in Alberta, Canada before completing a three-year residency in Equine Surgery and a MS in Clinical Sciences at Colorado State University, resulting in board certification in the American College of Veterinary Surgeons. She then worked as a Visiting Assistant Professor at Oklahoma State University before joining the University of California, Davis, where she is an Associate Professor in Clinical Equine Surgical Emergency and Critical Care. Dr. Dechant completed a ACVECC fellowship at UC Davis and is board certified in the American College of Veterinary Emergency and Critical Care. Special interests include improving diagnosis and management of emergency and critical care patients.
