Description

Unit 8 :Sleep Case Kevin
Positional sleep apnea is extremely important. Evaluation of the degree of severity of sleep apnea depending on body position is an important part of diagnosis. Most patients are worse on their backs (supine position).

Dual therapy involving both oral appliances and CPAP are sometimes necessary to prevent snoring and sleep apnea in some patients.

Objectives

On completion of this unit the clinician will understand:

Male overweight patient age 53. History of two heart attacks, bypass surgery, extreme daytime sleepiness, loud snoring, CPAP intolerant. Case of positional sleep apnea. The sleep study gives data regarding how much time the patient sleeps on their sides or supine. Severe sleep apnea on his back, mild sleep apnea on his side. treatment was two fold. On completion of this unit the clinician will understand:

Male age 67, high blood pressure, severe daytime sleepiness, snoring, severe sleep apnea. CPAP was not effective in controlling his sleep apnea.. Oral appliance therapy not effective in preventing sleep apnea. Pretreatment hospital sleep study and post treatment home sleep study proved that dual therapy involving the oral appliance (Dorsal Appliance) and the CPAP worn together successfully reduced both the snoring and sleep apnea.
On completion of this unit the clinician will understand:

Female age 40, obese (BMI over 30), loud snoring, severe sleep apnea, deviated septum, witnessed apneic events, cannot tolerate CPAP. Full sleep records including extra-oral and intra-oral photographs, initial consultation form, Epworth Sleepiness Scale, study models, pretreatment hospital sleep report diagnosed by a sleep specialist, TMJ Health Questionnaire, Range of Motion. Summary of radiographs including cephalometric, panorex, nasal x-ray and tomograms of the TMJ. J
