Description

The root canal system is highly complex making cleaning during root canal treatment challenging. This course reviews the issues that causes problems during the cleaning of root canals including how most files are round limiting their potential to scrape oval shaped canals, fins off the main canals, communications and isthmuses between the canals. Many adjunct techniques have been tried to compensate for these shortcomings including. high concentration of NaOCl, EDTA, ultrasonication or by lasers technologies. The second half of this course shows how the XP-endo Finisher (XPeF) files are designed and produced using the principles of memory shape of NiTi alloys and will transform to a specific shape at body temperature inside the root canal. At maximum austenitic phase (35oC) the file has a semi-circular shape with a 3mm diameter to enable it to transform into any canal shape and reach irregularities, fins, resorptive areas etc…. It is designed to be used with irrigants after initial root canal instrumentation with the aim of removing vital and/or necrotic tissues, dentinal debris accumulated during instrumentation and smear layer. The file does not require any specific motor or irrigation system. Micro-CT studies have shown remarkable effectiveness for this finishing file.

Objectives
Understand the role of mechanical instrumentation in controlling intra canal infection and facilitate the effects of irrigation and intracanal medication in disinfecting the root canal. Understand the technological challenges and possibilities in endodontic instruments to predictably remove intra canal infection. Present the physical and clinical characteristics of the new XP-endoFinisher files. Demonstrate the clinical use of XP-endo Finisher files. Present preliminary micro CT studies showing its effect of removing soft and hard tissues left behind or produced during the instrumentation.

Dr. Martin Trope was born in Johannesburg, South Africa where he received his BDS degree in dentistry in 1976. From 1976 to 1980 he practiced General Dentistry and Endodontics. In 1980 he moved to Philadelphia to specialize in Endodontics at the University of Pennsylvania. After graduating as an Endodontist he continued at the University of Pennsylvania as a faculty member until 1989 when he became Chair of Endodontology at Temple University, School of Dentistry. In 1993 he accepted the JB Freedland Professorship in the Department of Endodontics at the University of North Carolina at Chapel Hill, School of Dentistry. Named in honor of one of the founding fathers of Endodontics, the Freedland Professorship recognizes significant contributions to the specialty. Dr. Trope is currently Clinical Professor, Department of Endodontics, School of Dental Medicine, University of Pennsylvania and in private practice in Philadelphia. Dr. Trope is actively involved in the development, design and promotion of new technological advancements in Endodontics. Presently he acts as Clinical Director for Brasseler USA. Dr. Trope has created Next Level Endodontics, continuing dental education program, combining his extensive background of the theoretical world of academics with his considerable experience in private practice.
