Description

CHAPTER 2. A CLINICAL PERSPECTIVE ON WAVEFRONT SCIENCE (56 pages)
Description:
This discussion addresses the principles and practical applications of adaptive optics which introduce new concepts in vision care including root mean square deviations, Zernike Polynomials, point spread function and many more.
Course Outline:
I. Clinical Perspective on Wavefront Science
A. Adaptive Optics
1. Optics used by astronomers and physicists
2. Optics of wavefront aberrometry
B. Root Mean Square (RMS) Deviation Unit
1. Measures down to 0.05µ
2. Can measure higher order aberrations at > 3µ
C. “Super Vision”
1. Potential retinal resolution = 20/8
2. Limit of human vision
D. Lower Order Aberrations and Radial “Orders of Vision”
1. Piston (0 order)
2. Tilt (or prism) – 1st order aberration
3. Defocus (sphere) – 2nd order aberration
4. Cylinder – 2nd order aberration
E. Zernike Polynomials
1. Measured in RMS
2. Measure 2-dimensionally or 3-dimensionally
3. Measures higher order aberrations (HOAs)
4. Can measure the circle of least confusion
5. Zernike is the “map” of vision that comes through our pupil
a. Dilated vs. constricted pupil changes Zernike
b. Day vs.night vision

F. The Zernike “Pyramid” and Higher Order Aberrations
1. Coma and trefoil (3rd order aberrations)
2. Spherical aberrations and quatrefoil (4th order aberrations)
3. Secondary coma (5th order aberrations)
4. Secondary spherical (6th order aberrations)
5. 3rd and 4th order most important to human vision
6. Pyramid can go to 128, 256, 512 orders or greater
a. Anything above 5th is effected by the tear film variability
b. “Retinal noise” – not vision
G. Point Spread Function (PSF)
1. Black spot on white background
2. Reconstruction of Zernike Polynomial
3. Assimilates the Snellen chart (“black letters on white background”)
4. “But it’s not clear” is a measure of HOAs
a. Coma (3rd order)
1) “I

Objectives

Objectives
1. To introduce the concepts associated with adaptive optics;
2. To explain at a clinical level, adaptive optics;
3. To apply adaptive optics in a practical manner to clinical vision care

Wavefront has been evolving as a clinical science in eye care for the past 20 years now. It started with refractive surgery in the 1990s when I became involved with it and it has been growing rapidly as a vision and eye care diagnostic science as well ever since. I published this textbook in 2009 with the goal of converting the complex mathematical and optical physics of wavefront science into understandable clinical, diagnostic and non-surgical and surgical corrective approaches for day-to-day practitioners’ use. The story is told over 6 chapters in a casual, ‘user friendly’ style with extensive graphic support. It’s actually an enjoyable – almost a fun read. An extensive 30 page Subject Index is available on this website at no charge for referencing and/or downloading (as a pdf file) at the program entitled Wavefront Science in Vision and Eye Care: SUBJECT INDEX (LINK: http://wco.easyshare.education/by/dr-louis-catania/128075/wavefront-science-in-visio-and-eye-care-subject-index). And finally, under Additional Course Notes section, the Front Matter from the textbook is available. It includes:
> Table of Contents (general and detailed);
> Preface;
> Acknowledgements;
> Introduction

Author Bio:
> Internationally acclaimed clinical educator, author;
> Recognized expert in anterior segment, corneal disorders, refractive surgery and new eye care technologies;
> With Nicolitz Eye Consultants, a multispecialty ophthalmology group in Jacksonville, FL;
> Does clinical research on numerous eye care technologies;
> Voted one of 10 most influential optometrists of 20th century by the Review of Optometry Journal and 20/20 Magazine;
> 2013 Distinguished Service Award from the Ocular Surface Society;
> 2014 Distinguished Service Award from the American Academy of Optometry;
> 2015 Primary Care Optometry News, “Pioneer in Optometry” Award
