Description

As health care professionals, we strive every day to provide patients with safe, effective, beneficial care in each practice setting. It is when patients transition between settings that problems are most likely to occur, due to a complex health care system with no clearly defined responsibilities to facilitate effective communication. The National Transitions of Care Coalition (NTOCC) was formed in 2006 to bring together thought leaders, patient advocates, and health care providers to improve care coordination and communication when patients are transferred from one level of care to another. The transitions in care include patients moving within the hospital, and being discharged to home, assisted living, or a skilled nursing facility. This presentation will discuss NTOCC initiatives, responsibilities for effective patient transitions, and tools to reduce transitional problems to improve patient safety.
