Description

Documentation has always been an indispensable component of the healthcare delivery system. It is a record of many subjective, as well as objective, observations made about clients’ needs and conditions by the caregiver. It is a "firsthand" record, meaning that the person who gave the care or made the observation does the documenting. This documentation should, of course, remain confidential. It is also a legal document that may be admitted in a court of law if a client or their family uses legal means to dispute the quality of care received by the client.
