Description

In this recorded session, nurse practitioner Kim Newlin reviews the Medicare penalties in place for hospitals who exceed the national average for readmission rates for certain cardiac diagnoses and provides practical tips for improving hospital to home transitions.

Objectives

Kim Newlin is currently practicing as a Cardiovascular CNS and ANP as well as Clinical Manager of Cardiac and Pulmonary Rehab and the Cardiovascular Transitions team at Sutter Roseville Medical Center. Kim continues to see patients at the hospital’s Heart Health Clinic, which she helped establish five years ago, while teaching a variety of cardiovascular based classes to the nursing staff. She has been the President of the local Preventive Cardiovascular Nurses Association (PCNA) chapter for over 10 years and serves on PCNA’s Board of Directors.
