Description

CHAPTER 2. CLINICAL CONSIDERATIONS ON ANTERIOR SEGMENT PATHOLOGY AND IMMUNOLOGY (90 pages)
Description:
An in-depth discussion of the relevant aspects of clinical immunology regarding anterior segment pathology. A comprehensive approach to inflammation will give practitioners a clear understanding of anterior segment therapeutics
Course Outline:
I. Current Concepts, Terminology,and Abbreviations
A. Definitions
1. The anterior segment of the eye may be consid¬ered the eyelids and adnexa,
and the front (anterior) one third of the globe (eyeball)
2. Pathology is the study of abnormal structures, tissues, or functions that may
produce disease
3. Immune system
a. A regulated defense system protecting the body ("self") against antigens ("immuno¬gens"), or foreign ("nonself") substances or stressors
b. Natural or innate immunity
c. Acquired immunity
B. DEFENSE MECHANISMS OF THE ANTERIOR SEGMENT
1. Prevention of negative stimulation (toxic, me¬chanical, physiological), more than any de¬fense system, is the foremost means of pro¬tecting and defending our bodies and the anterior segment of our eyes
2. Normal anatomy and physiology serves as the primary defense systems
3. Acquired Immunity
C. The "Clinical Immune Matrix"
Afferent Arc Stage
Central Processing Stage
Efferent Stage
Regulation Stage
Dysregulation
II. Inflammation
III. Clinical Theories on Variable Immunopathological Responses
A. "Most Things Get Better by Themselves"
B. "Sometimes Things Don't Get Better"
1. Causes
A. Definitions
B. The Inflammatory Cascade
1. Acute Versus Chronic
2. Control Mechanisms
3. Vasodilation ("Rubor" and "Calor")
4. Increased Vascular Permeability
(Edema or "Tumor" and "Dolor")
5. Infiltration
6. Ulceration
7. Loss of Function ("Functio Laesa")
8. Repair, Healing, and Scarring
(Cicatrization)
C. Hypersensitivity/Allergic Respo

Objectives

Objectives
1. To introduce an understandable model of clinical immunology
2. To define and describe inflammation at the clinical level

Primary Care of the Anterior Segment, 2nd Edition (Textbook)
Published in 1995 and reprinted numerous times through the early 2000s, this book is considered one of the classic, definitive textbooks for comprehensive anterior segment diagnosis, treatment and management. All the clinical information is organized in the SOAP (Subjective, Objective, Assessment, Plan) format for quick referencing and practical use. The textbook is out of hardcopy print and I am making it available electronically through this website. Each chapter (8 total) provides immediate, accessible, "in the chair" information for clinical practitioners. The textbook also includes 7 appendices referenced throughout the respective chapter programs on this website. The appendices are included at the end of each chapter program. All chapter programs include a set of self-assessment questions and answers (in Appendix 7). An extensive 20 page Subject Index is available on this website at no charge for referencing and/or downloading (as a pdf file) at the program entitled Primary Care of the Anterior Segment, 2nd Edition: SUBJECT INDEX (LINK: http://wco.easyshare.education/by/dr-louis-catania/124599/primary-care-of-the-anterior-segment-2nd-edition-subject-index). And finally, under Additional Course Notes section, the Front Matter from the textbook is available. It includes:
> Table of Contents (general and detailed);
> Preface;
> Acknowledgements;
> Introduction
